🧨 Defect Management Metrics Every QA Professional Should Know (Part 5)🧨

Most teams track defect counts. But mature QA organizations track how well the defect process works—because the biggest cost isn’t the bugs themselves, it’s the rework, noise, and backlog growth that slow delivery and reduce release confidence.
In this article, we cover four defect management metrics that reveal process quality and delivery stability:

  • Defect Reopen Rate
  • Defect Rejection Rate
  • Open Defects Change Rate
  • Defect Removal Efficiency (DRE)

1) Defect Reopen Rate: Are our fixes really fixed?

A reopened defect is a defect that was marked Resolved or Closed, but later reopened because:

  • the fix was incomplete,
  • the problem still exists in certain scenarios,
  • the validation missed edge cases,
  • or acceptance criteria were misunderstood.

Reopens create direct waste: the team spends time fixing and verifying the same issue more than once.

📐How to measure
Most tools (Azure DevOps, Jira, Bugzilla) allow status history tracking.
A common formula is:

Reopen Rate (%) = Reopened defects ÷ Resolved defects × 100

How to interpret

  • Low reopen rate = strong fix quality + strong verification
  • High reopen rate = rushed fixes, weak regression, unclear acceptance criteria, or poor RCA

Watch for patterns:

  • same module repeatedly reopened,
  • same defect types reopened,
  • reopens concentrated in certain developers or components.

Example KPI zones

  • 🟢 Green: <5%
  • 🟡 Amber: 6–10%
  • 🔴 Red: >10% (often stricter in finance/healthcare)

Recommended actions

  • Tighten acceptance criteria and “Definition of Done”.
  • Add mandatory code reviews for high-impact fixes.
  • Expand regression and edge-case coverage.
  • Perform RCA on repeated reopen categories.

2) Defect Rejection Rate: Are we generating noise instead of signal?

Defect rejection rate tracks how many submitted defects are rejected during triage because they are:

  • duplicates,
  • “not a bug” / expected behavior,
  • non-reproducible,
  • out of scope,
  • won’t fix.

A high rejection rate wastes time, increases friction between QA and dev, and reduces confidence in defect reporting.

📐How to measure:

Rejection Rate (%) = Rejected defects ÷ Submitted defects × 100

How to interpret

  • Low rejection rate = high-quality defect reporting and shared understanding
  • High rejection rate = unclear requirements, inconsistent test data, weak bug descriptions, or misalignment on expected behavior

Example KPI zones

  • 🟢 Green: <10%
  • 🟡 Amber: 11–20%
  • 🔴 Red: >20%

Recommended actions

  • Improve defect reporting templates (steps, actual vs expected, evidence, environment, logs).
  • Clarify acceptance criteria and expected behavior earlier.
  • Improve test data consistency.
  • Provide coaching where rejection is concentrated.

3) Open Defects Change Rate: Is the backlog growing or shrinking?

This metric tracks how the open defect backlog changes over time, often week to week.
It reflects the balance between:

  • new defects being reported, and
  • defects being resolved and closed.

It acts like an early warning system:

  • stable/decreasing backlog = healthy control
  • increasing backlog = process bottlenecks or too many issues entering the system

📐How to measure
Track open defects at the end of each period and compare it to the previous one (optionally as %).

How to interpret
An increasing trend can indicate:

  • delayed testing of fixed items,
  • slow verification/closure,
  • poor fix quality,
  • or defect inflow exceeding capacity.

Example KPI zones

  • 🟢 Green: ≤0% change (stable or decreasing)
  • 🟡 Amber: +10–15%
  • 🔴 Red: >+15%

Recommended actions

  • Prioritize defect resolution over new feature work (especially in red).
  • Remove bottlenecks: delayed QA validation, delayed PO closure, unclear ownership.
  • Focus on prevention: reduce incoming defects via better reviews and early testing.

4) Defect Removal Efficiency (DRE): How efficiently are we clearing defects?

While open defects change rate shows the trend, Defect Removal Efficiency provides a clean efficiency snapshot.

📐How to measure

DRE (%) = Defects resolved ÷ Defects reported × 100

This answers:

Are we resolving defects faster than they arrive?

How to interpret

  • High DRE = strong throughput and control.
  • Low DRE = backlog growth, capacity mismatch, process inefficiency.

KPI example

  • 🟢 Green: >90%
  • 🟡 Amber: 75–89%
  • 🔴 Red: <75%

Recommended actions
Same as backlog control:

  • increase fix throughput (capacity, focus, triage).
  • reduce inflow through prevention and early quality gates.
